Substance abuse Evaluation (S.A.E.)

1. Complete the Required SALCE Survey

Use the provided link to access the SALCE Interactive Survey
Use the last 4 digits of your Social Security Number as your ID
Submit the survey—it will be sent directly to your evaluator

2. Book Your Appointment

Select your preferred date and time
Submit payment via credit/debit card
 

3. Check Your Email

First email: Booking confirmation + receipt
Second email (within 24 hours): Secure client portal login link
 

4. Access the Client Portal

Log in through the secure link
Complete all intake forms
Upload a clear photo ID
 

5. Call the Office

Call us at (832) 649-8038
We’ll confirm your paperwork and provide your Zoom meeting link
 

6. Attend Your Virtual Evaluation

Meet with a Licensed Professional (30–45 minutes)
 

7. Recommendation Sent to Referring Party

Evaluation results will be sent directly to the court, CSO, or referring agency within 48 hours

Answer all questions honestly and accurately for the most reliable results.

Your responses will be automatically forwarded to the evaluating counselor for review.
 

This step is required before your scheduled evaluation can take place.
